Almost every patient who walks in with a flaky scalp assumes it’s “just dandruff.” Sometimes it is. But scalp psoriasis and fungal scalp infections look similar on the surface and need completely different treatment which is why over-the-counter anti-dandruff shampoos sometimes don’t work no matter how long you use them.
Dandruff (Seborrheic Dermatitis)
Fine, white or yellow flakes, often with an oily scalp and mild itching. Worsens with stress, humidity, and infrequent washing. Usually responds well to medicated anti-dandruff shampoos (ketoconazole/zinc pyrithione based).
Scalp Psoriasis
Thicker, silvery-white scales with well-defined red patches underneath. Can extend beyond the hairline onto the forehead or behind the ears. Often runs in families and may appear alongside psoriasis elsewhere on the body. Doesn’t respond to regular anti-dandruff shampoo needs prescription treatment.
Fungal Scalp Infection
Patchy hair loss combined with flaking, sometimes intensely itchy with small pustules. Spreads with shared combs, pillowcases, or helmets. Needs anti-fungal treatment, not steroid-based products steroids can actually worsen fungal infections.
Why Self-Treatment Often Fails
Most anti-dandruff shampoos target seborrheic dermatitis specifically. If what you actually have is psoriasis or a fungal infection, months of the wrong shampoo will show little to no improvement which is exactly the pattern we see in patients who eventually come in frustrated.
